Individuals in Dual Eligible Special Needs Plans (D-SNP) have greater clinical and social needs than members of standard Medicare Advantage plans. This sizable population of about 5 million nationwide, who are entitled to Medicare combined with Medicaid, are often difficult to engage. There is also the opportunity for higher value engagement as a result of the D-SNP populations greater clinical utilization and increased social needs.

In this webinar, we will cover how the Wider Circle Connect for Life program significantly improves engagement, retention and gap closure.
